Heart failure is responsible for 100,000 hospital admissions and one million inpatient days each year in the NHS, with 80% of patients only diagnosed after an emergency admission. Our national programme aims to change that. Over the next two years, 11 health innovation networks will work with their local systems to transform pathways to support earlier diagnosis in primary care, reduce emergency admissions and enhance care and improve patient outcomes through new innovations, digital tools, community care and access to evidence-based treatment. Meet some of the innovators we're working with with innovations to support CVD and heart failure pathway transformations including: 👋 Tommy Parker KiActiv® 👋 Dr Nick Hartshorne Evans DUniv BEM Pumping Marvellous Foundation 👋 Steve Roest PocDoc Find out more: https://lnkd.in/eRrz8gah #NHS #HeartFailure #HealthInnovation #Prevention #DigitalHealth
